कृपया एडमिशन फॉर्म भरने से पहले इन निर्देशों को ध्यान से पढ़ें। / Please read these instructions carefully before filling the admission form.
फॉर्म भरने के महत्वपूर्ण निर्देश: / Important Instructions for Filling the Form:
- सभी आवश्यक फ़ील्ड्स (जिन पर * लगा हो) भरना अनिवार्य है। / All required fields (marked with *) are mandatory.
- बच्चे का पूरा नाम जन्म प्रमाण पत्र के अनुसार ही लिखें। / Write the full name of the child strictly as per the Birth Certificate.
- जन्म तिथि सही-सही भरें, यह प्रवेश के लिए पात्रता निर्धारित करेगी। / Enter the Date of Birth correctly, as it will determine eligibility for admission.
- माता-पिता का नाम उनके आधार कार्ड के अनुसार लिखें। / Write the names of parents as per their Aadhaar Card.
- संपर्क जानकारी (मोबाइल नंबर, ईमेल) सही और चालू हालत में दें। / Provide correct and active contact information (Mobile Number, Email).
- आधार कार्ड पर पंजीकृत मोबाइल नंबर अनिवार्य है। / Mobile number registered on Aadhaar Card is mandatory.
- कृपया सभी आवश्यक दस्तावेज (बच्चे की फोटो, आधार कार्ड, जन्म प्रमाण पत्र आदि) स्कैन करके तैयार रखें। / Please keep all necessary documents (Child's Photo, Aadhaar Card, Birth Certificate, etc.) scanned and ready.
- अपलोड किए जाने वाले दस्तावेज़ JPG, PNG, या PDF फॉर्मेट में होने चाहिए और प्रत्येक की अधिकतम साइज़ 5MB होनी चाहिए। / Documents to be uploaded must be in JPG, PNG, or PDF format and each file size should not exceed 5MB.
- फॉर्म जमा करने से पहले भरी हुई सभी जानकारी को ध्यान से जांच लें। / Carefully review all the information filled before submitting the form.
- घोषणा (Declaration) को ध्यान से पढ़ें और सहमत होने पर चेकबॉक्स पर टिक करें। / Read the Declaration carefully and tick the checkbox if you agree.
आवश्यक दस्तावेज़ (अपलोड के लिए): / Required Documents (for Upload):
- बच्चे की पासपोर्ट साइज़ रंगीन फोटो / Passport size colour photograph of the child
- बच्चे का आधार कार्ड / Child's Aadhaar Card
- बच्चे का जन्म प्रमाण पत्र / Child's Birth Certificate
- पिता का आधार कार्ड / Father's Aadhaar Card
- माता का आधार कार्ड / Mother's Aadhaar Card
- (यदि लागू हो) जाति प्रमाण पत्र / Caste Certificate (if applicable)
- (यदि लागू हो) विशेष आवश्यकता प्रमाण पत्र / Special Needs Certificate (if applicable)
- (यदि लागू हो) पिछले स्कूल की टीसी (Transfer Certificate) / Transfer Certificate (TC) from the previous school (if applicable)
